USTEP 一時渡航届 Temporary Leave Form

本フォームは出国の1週間前までに提出してください。
※提出後に旅程に変更等が生じた場合には直ちに国際交流課に連絡すること。また、帰国後には国際交流課(ustep-office.adm@gs.mail.u-tokyo.ac.jp)にメールでその旨を伝えること。
Submit this form at least one week before you leave Japan.
*If there are any changes in your itinerary after the submission, notify the International Exchange Group of the change immediately. On your return to Japan, inform the International Exchange Group(ustep-office.adm@gs.mail.u-tokyo.ac.jp) about your return by email.

新型コロナウイルス感染症の未収束の中、国際交流課としてはプログラム期間中の日本からの出国は勧めておりません。特別な事情により出国せざるを得ない場合には、本フォームにも記載されている確認事項について十分理解した上で本フォームを提出し、出国してください。なお、本フォームを提出せずに出国した場合には、即座にプログラム中断等の措置をとることがあります。
With the COVID-19 pandemic still being uncontained, the International Exchange Group does not recommend students leaving Japan during the exchange program. If you must leave Japan for unavoidable reasons, please read all the points listed below in this form carefully, understand them, and submit this form before leaving. If you leave Japan without submitting this form beforehand, we will take immediate response, such as the cancellation of your enrollment in the exchange program.

確認事項 Points to Note	以下、すべての確認事項にチェックを入れてください。Please check all the boxes of Points to Note below. 一時渡航に際し、本学からの再入国支援は一切受けられない。出国後に水際対策等に変更があり、日本へ入国ができなくなった場合はプログラムを中断することになる可能性が高い。UTokyo will not provide any reentry support for your temporary leave. If there are any changes in the Japanese border measures after you leave Japan which results in the denial of your reentry, there is a high chance that your enrollment in the exchange program will be cancelled. 隔離期間等が発生し、それにより授業履修に影響がある場合は必ず自身で教員に連絡をとること。また、授業を欠席すること等により履修を断られたり、単位がつかなかったりする場合などいかなるケースも学生は不服を申し立てることはできない。※国際交流課では各教員の連絡先は把握していません。また、国際交流課は授業や成績のことに関しては一切介入しません。You are responsible for contacting your instructors in the event you are required to be quarantined thus affecting your course work. If the instructors deregister you from their courses or do not give credits for the courses due to your absence from the classes, you will not be able to claim them.*The International Exchange Group does not have contact information of each instructor. Furthermore, we will not intervene in matters concerning classes or academic results. 入国不可能となった場合、滞在先に置いている自身の荷物については、梱包や郵送を日本にいる友人等に頼むなどして、母国へ送ってもらえるように自身で手配する。If you are not able to reenter Japan, your personal belongings left at your accommodation in Japan must be sent back to your home country. Please arrange the packing and delivery of your belongings by asking for help from your friends in Japan. 奨学金受給者の場合、プログラム中断により全額返金等の可能性が生じる場合がある。If you are a scholarship recipient, you may be required to repay all the scholarship you have already received in the event that your enrollment is cancelled.
